Getting Started

1. Accept the invitation from CampDoc to create an Account
2. Navigate to your CampDoc questionnaire page at app.campdoc.com
3. Log In
4. Click "Scouting Medical Form"

Part A: Informed Consent, Release Agreement, and Authorization

Participant Authorization

1. Click "A - Informed Consent, Release Agreement, and Authorization"
2. Click the "Your Name" box and enter the Participant's name
3. Click "Accept Authorization"

Parent/Guardian Authorization (if participant is under 18)

First Parent/Guardian:

1. Click the "Your Name" box under Parent/Guardian section
2. Enter the 1st parent's name
