Medical Form Requirements

🩹CampDoc Medical Form Registration Guide

Every participant (Scouts AND adults) must complete their health and medical information in CampDoc, the Council’s secure online system. All profiles must be completed and submitted before arrival at camp.

Required CampDoc Sections:

Part A – Informed Consent, Release Agreement, and Authorization (completed and signed)

Part B – General Health History (completed and signed)

Part C – Pre-Participation Physical (physical exam within 12 months of your camp end date, signed by a physician)

Tetanus vaccination must be within 10 years

The doctor's signature is required on Part C

Copy of current health insurance card


Health Lodge & Medical Staff

Camp Hi-Sierra has a Health Lodge staffed with trained medical personnel throughout the camp season. The Health Lodge provides:

First aid and basic medical treatment, medication administration, health screening upon arrival, emergency response coordination, and health consultations.

Sunday Check-In: After registering at the camp office, all participants proceed to the Health Lodge for medical screening. Medical forms are reviewed and any medications are collected at this time.
